Parents/Guardians are required to complete a Return to Educational Facility Parental Declaration for their child prior to returning to school after any absence.
Two versions of this form are attached below (one printable and one which may be completed digitally). The completed form may be submitted via email to your child’s class teacher or a paper version may be sent in with your child on the morning they return.
A further option is to confirm via email that you have viewed the form online Return to Educational Facility declaration form.pdf (hpsc.ie) and confirm the declaration by including your name and your child’s name in the email to your child’s teacher.
